Clinical work
1. Current Activities in the CEMNIS
In December 2016, I created and head the non-invasive neuro-modulation center of Strasbourg or CEMNIS. This project is based on the researches I conducted since 2002, which enabled me to develop a complete technical suite for pre-interventional brain mapping and personalized treatment of refractory psychiatric pathologies.
The CEMNIS is a consultation and day-care unit of 10 beds dedicated to the evaluation and treatment of neuro-psychiatric pathologies resistant to usual treatments, mainly depression, but also psychosis and chronic pain. The CEMNIS essentially propose non-invasive neuromodulation therapies mostly using robotized transcranial magnetic stimulation or TMS. What make the CEMNIS unique world-wild is the use of functional MRI guided personalized targeting of the TMS precisely targeted thanks to a robotic positioning device that we developed and tested with the iCUBE laboratory. More exceptionally, CEMNIS offers other innovative and / or at-risk therapies (CBASP, KETAMINE infusion, MAOI and association with LEVOTONINE or SSRI ...). I carry out about half of the 1000 consultations per year and I am responsible for the programming of the robot and half of the monitoring of the 2000 days-hospitalization per year.
2. Collaborations
I continue a close collaboration with the pole of psychiatry, mental health and addictology. I participate in the schizophrenia expert center headed by Professor Fabrice Berna and at the service meeting. On the other hand, I participate in the clinical and research meetings on mood disorders with Professor Gilles Bertschy and his team.
3. The CEDRA
Finally, I am the leader of the Alsace’s expert center for resistant depressions of Alsace or CEDRA project which should start in the beginning of 2018. The CEDRA brings together the CEMNIS and the psychiatric, mental health and addictology services of the University Hospital of Strasbourg and the 8-9 Psychiatric services of the Rouffach Hospital Center. It will offer expertise in the management of resistant depressions, residual, disabling symptoms such as cognitive and sleep disorders, the management of treatment side effects and the prevention of recurrences. The CEDRA will coordinate the research on affective disorders throughout the Alsace region and will apply to become part of the network of expert centers of the “Fondation FondaMental”.
Teaching duties
I contribute in courses proposed by the Institute of Physiology in the Faculty of Medicine of Strasbourg and paramedical schools. I also take part in the training of psychiatrists and residents on the pathophysiology and psychophamacologie of psychiatric illnesses (in charge of two seminars). Finally I participate in the teaching of the Master of neuroscience where I am in charge of the forthcoming module in neuroimaging and in the teaching of the master in imaging, robotics and bioengineering

1.3. Clinical teaching and therapeutic in psychosis : the club psychose.
Since November 2005, the club gather twice a month in psychiatrist and researchers. Meetings alternate between article presentation and clinical cases. You will find archives and program on this website (since December 2007).

The Circle of Excellence in psychoses (CEP) is an European intellectual community. We seek to promote a differentiated approach in medical care and research on psychosis. This requires us as a better definition of disorders which we face, up to the delineation of diseases. The most validated classification system, the Wernicke-Kleist-Leonhard system is unfortunately unknown, the CEP promotes education both in theory (seminar in differential psychopathology) and in practice (see case report). Ultimately, we aim to find biomarkers in these refined phenotypes.

Research
1. Research performed up today
My research activity is carried out so far in the context of the U405 and U666 INSERM ("Clinical and Experimental Pathophysiology of schizophrenia"), in collaboration with the LINC UMR 7237 CNRS ("Laboratory inmagerie and cognitive neuroscience "). I initially focused on the mastering of the techniques of functional exploration of the central nervous system, signal processing derived therefrom, and the development of cognitive originals paradigms. I have developed a wide range of techniques (multimodal imaging), from indirect measures of brain activity (fMRI BOLD and ASL SPECT) to more direct measures (EEG, MEG or magnetoencephalography). This work has resulted in technical publications, but also fundamental results in healthy subjects in the field of vision, attention, vigilance and physiology of BOLD signal (contrast used functional MRI). In the field of pathology I could describe the disturbances of functional connectivity in schizophrenia. I was able to show their direct implication in symptoms such as hallucinations. Finally I developed an interventional technique: The transcranial magnetic stimulation (TMS). I developed a coherent imaging-analysis pipeline to reliably know the areas involved in symptoms such as hallucinations, ruminations and psychomotor retardation. Classical stimulation techniques are insufficiently precise and can not deliver an homogeneous stimulation. Thus I intitiated and contributed to the development of a robotic system for this purpose with the robotics team of LSIIT, UMR 7005 CNRS (Strasbourg ). I currently performing its validation. And the device will be release this year for clinical work.
2. Research project
As described elsewhere on this site, pathophysiological research on schizophrenia is non sens as it can not be considered as a disease. This observation forced me to change my of research team as resistance to this evidence was strong in the former one. I therefore integrates the IMIS team (imagernie multimodale et intégrative pour la santé) of the Icube laboratory (laboratoire des sciences de l'ingénieur, de l'informatique et de l'imagerie, UMR CNRS 7357). This is the team where all the developments mentioned above have been made. I coordinate the neuro-crypto group for highlighting "cryptic" (hidden) anomalies in neuropsychiatric disorders.
Thus, future research will focus on physiological disorder of finer phenotypes (Wernicke, Kleist et Leonhard or WKL school) and catatonic symptoms. It has already been possible to show significant differences in terms of phenotype between cycloid psychoses and emotional paraphrenia.
But beyond this attempt to characterize real diseases in the field of psychosis, it is also important to expand our armamentarium abd to use it more rationally. As the pathophysiology or etiology of psychiatric disorders are unknown, there is no way to find a physiopathological or ethiological cure. But we already have symptomatic treatments, and it is possible to streamline and personalize their use. Indeed, it sometimes takes many tries before finding the right medication in certain "schizophrenia" or in some depressions. Currently trying these last between 2 and 8 weeks. Yet it is possible to characterize the specific functional anomalies in a patient and even the effect of a first dose. It should be theoretically feasible to adapt within the first days instead of waiting several weeks for a response. It would even be possible to choose the best treatment based on functional abnormalities observed in a given patient. Finally, some symptoms are too rare for specific treatments to be developed by the industry (pseudo-OCD, mannerisms, proskinesia, negativism ...). Robotized rTMS may be a way to treat these orphan symptoms in the same way as already done for depression and refractory hallucinations.
